What is everyone using to have personal lines clients input information into online web forms? 

Is anything available that dumps into a rater?  Or applied?  Trying to make it easier for clients and us to fill out personal lines questionnaires.

You could use Google Forms and have it dumped into A Google Spreadsheet.  Have the spreadsheet notify someone via email each time it's updated.

That's one option, at least.
