driver data not pulling into report

We run a search on Personal Auto Policy applications to send birthday cards. I noticed some more blanks than usual and decided to look into it. Turns out the First and Last name were evident in the system, just not being pulled into the excel spreadsheet. So far i've looked at the following:
It's not the same carrier, in case it was a download issue.
The name does appear if you preview the application into a PDF
The name also appears if you print the application.
I've reindexed reports.

Has anyone seen anything like this or does anyone have any thoughts? We are on 12, (yes 12 I hope to update this month).

Since I'm on an older version of TAM, the only thing I can think of is that was there something in another criteria field that was not set to "all" or was limited in some way?

Lynne - is this a saved report? or just one that is created when you run it.
